The World Diabetes Day is marked every  year on 14 November, the birthday of  Sir Frederick Banting, who co- discovered the insulin along with Charles Herbert best in 1922.

Recently Victor Hospital, Margao successfully treated a 90-year-old suffering from jaundice caused by liver cancer with ‘Y’ stenting. Since 8th November is commemorated as International Radiography Day, Radiologists and Radiographers contribute to the continuum of healthcare in a variety of ways. A key objective of the program is to increase public awareness of the role that radiology plays in the safety and treatment of patients.

As part of its commitment to excellence, Victor Hospital would like to congratulate its Radiology team on a successful ‘Y’ stenting performed on a 90-year-old female cancer patient. The woman presented with severe jaundice and MRI’s and CT scans performed on her revealed that she had liver cancer that was blocking the biliary duct. In addition, she developed a liver infection as well.

Dr. Franzil de Miranda, Consultant Radiologist – Victor Hospital states that “Percutaneous trans hepatic biliary stenting is a form of pin-hole surgery that can be performed under local anesthesia for cancer-related jaundice. As a result, her septicemia, which was unresponsive to antibiotics, was cured”.

Considering her advanced age and her fitness, surgery and general anesthesia would not be the right option for her due to the risks involved therefore the decision was made to insert a stent into the left biliary duct using an endoscope. In the next step, the interventional radiologists performed a procedure known as PTBD (percutaneous trans hepatic biliary drainage), which involved entering the blocked bile duct through the skin with just local anesthesia and draining pus from the liver itself. Through the previous stent, a metallic stent is inserted in a ‘Y’ configuration. A very important point to note is that both the jaundice, as well as the infection disappeared after the procedure had been completed. After a few days in the hospital, the patient’s appetite started to improve and she was discharged.


The Department of Oral and Maxillofacial Surgery at Victor Hospital is a well-known center providing a wide range of services for patients including treatment of facial injuries, diseases or abnormalities affecting the face, jaws and mouth, acquired, congenital and traumatic issues related to the jaws and facial region as well as developmental diseases of these areas.

Dr. Akshata Kotharkar stated that “Pain in the jaw or muscles on the side of your face near the ears, possibly accompanied by a popping sound or difficulty in jaw movement, may indicate that the person is suffering from TMD —Temporomandibular Disorders. One such 31-year old male patient who came to the dental OPD of the Victor Hospital had a lot of pain in the jaw on the right side along with severe headache and was not able to sleep and eat well. This patient was treated conservatively with splints which he had to wear in his mouth for around 16-17 hours a day for 2 months and with some medications he was free of the pain and happy”.

TMDs are problems related to the Temporomandibular Joint (TMJ), which is a hinge joint that connects the lower jaw to the bones of the skull in front of the ear on both sides of the head. The TMJ is a complex joint system connected by muscles and ligaments which lets us move our jaw up and down and side to side, which enables talking, chewing, yawning, etc.

TMDs can be either on one or both sides of the face. It mostly affects women more than men, and is more common among the age group of 20 to 40 year old’s. TMDs often cause varying amounts of pain or discomfort to the patient and jaws get “stuck” or “locked” in the open- or closed-mouth position, or the patient hears clicking, popping, or grating sounds in the jaw joint when the mouth is opened or closed or while chewing which may be accompanied by pain.

To diagnose TMDs, the doctor takes a detailed medical history and conducts a physical exam. One may require full face X-rays to view the jaw, TMJ and the teeth, to rule out other problems. Other tests like MRI to show if the TMJ disc is in the proper position as your jaw moves or CT scan to show the bony details of the joint, may also be conducted.

In some cases pain can be managed by medicines and pain relief exercises but may not be enough in cases with extreme pain. For such cases, non-invasive treatment may be required. “Failure to seek treatment can result in serious complications and permanent damage. Hence, timely treatment and follow ups with your doctor to reduce the risk of potential complications are very essential in case of TMD’s” says Dr. Akshata Kotharkar.

Victor Hospital’s Cardiothoracic Surgery department headed by Dr. Shanmukh Hiremath, a full-time Senior Consultant and Cardiothoracic Vascular & Transplant Surgeon, is one of the premier heart specialty centers in the country. Observations revealed that one patient, a 70 year old male, had a 15% ejection fraction which limited his daily activities. He would get chest pain with mild exertion and was at risk of one more heart attack which could have been fatal or even may have required a heart transplant which is extremely difficult.

The next patient had a left main coronary block with only one functioning kidney. Patients with weak hearts will have sub-optimal kidney function, so the single kidney was preserved during the operation and in the postoperative period.

The third patient presented with myasthenia gravis and hyperthyroidism which results in extreme muscle weakness, especially after anesthesia. Weaning these patients from ventilators is challenging.

Similarly, another patient, 72 years old, had both coronary blocks and a damaged aortic valve, requiring bypass surgery and valve replacement.

It is common for patients with many high-risk factors to avoid bypass surgery. All four patients recovered completely after receiving excellent care at Victor Hospital, including well-planned surgeries, pre-operative preparations and multidisciplinary postoperative treatment.

The camp was aimed at facilitating the availability of blood for the patients. A large number of people of the South Goa enthusiastically took part in the camp and donated blood. 80 units of blood  were collected.

Recently a 62 year old male walked into Victor Hospital’s emergency with acute chest pain which was central, radiating to the left arm and crushing in nature and was experiencing a heart attack with critical blocks in his heart. Generally after a heart attack, the heart becomes very weak and usually surgeon doesn’t perform surgery on a weak heart. Patient has to wait a minimum of 3 weeks to undergo the required surgery. But in this case the ECG was changing because blocks were persisting despite medication to break the clots threatening to cause another heart attack which could prove fatal.

Cardiothoracic Vascular & Transplant Surgeon, Dr. Shanmukh Hiremath, full-time Consultant – Victor Hospital decided to go ahead with a bypass surgery to prevent further heart damage & preserve heart function. The patient also had diabetes, high blood pressure & hypothyroidism. During the surgery he found the heart was very weak suffering with grossly reduced function. Doctor did the bypass surgery on a beating heart with arterial & venous grafts and treated the patient with optimal medications.  ‘As surgery was done on beating heart patient recovered soon and was breathing on his own & was removed from ventilator soon after surgery’ said Dr. Hiremath.
